giovedì, dicembre 16, 2004 :: ortho test ::it's back to TTSH for my test today! oh, i miss the good ole days there! haiz... nice environment, nice people... CANNOT be compared to a particular hospital in the west... poor rach's been stuck there for agessss! for the long case, i got TB spine! initially i was directed to the wrong bed.. and ended up clerking this rather young patient with a history of many falls and unsteady gait?! aiyoooo i was soo desperate by the time i got the super muddled history that i was ready to think about childhood hip disease leading to shortened leg! lolz... also i had a sneaky suspicion that he was PSY as well... given the really confusing history! then... my saviour came to get me to the CORRECT patient... lolz... turned out, the correct one also seemed a bit psy, having had a soujourn at u-know-where. he presented with lower lateral back LUMP! and claimed that it leaked pus?!?! and the pus came out in his stools?!?! pengz... then i was brought on a merry-go-round about prolapsed intervertebral disc and scoliosis... he claimed it was from birth... but it wasn't actually. aiyo... i almost didn't get the TB point despite repeatedly asking him abt med hx. until he let slip that he had some numbness after taking anti-TB drugs! by then i was combining hx taking with physical examination cuz time was FAST running out! for short case, got a pelvic x-ray showing a hip that was in bad condition probably due to RA/OA with protrusio acetabuli and the other side has a grade IV Garden's NOF #. the other two was this pressure mattress and an Ilizarov fixator. the latter two i didn't know, but anyway treat it as a learning experience. later on me and EW went nosing around and saw an interesting traction apparatus and got an MO to explain to us. hehe... here's a picture of the Ilizarov fixator:
